The College of Education, Sport, and Human Sciences is pleased to make these scholarship and fellowship awards on behalf of our alumni and donors, whose continued generosity makes all our awards possible.

How to Accept your Award(s)

Step 1

Complete the Acceptance Form(s) that are applicable to you:

  • The CESHS Scholarship Acceptance Form is for any CESHS Scholarship, except the RH & Jane Logan Scholarship. Complete the CESHS Scholarship Form once for each individual scholarship you received.

  • The Logan Acceptance Form is only for those who were awarded the RH & Jane Logan Scholarship.

    If you were awarded a Logan and something else from CESHS, you must complete both acceptance forms.

Step 2

Complete the Scholarship Recipient Donor Survey

  • To receive your scholarships from CESHS, you also must complete this survey. In lieu of a thank you letter, we’re requiring this survey to aid us in creating a thank-you document for the donors/stewards of your award. You only need to complete the survey ONCE per school year, even if you get more than one CESHS Scholarship or Logan Scholarship.

Here are the questions the Scholarship Recipient Donor Survey asks you, in case you want to prepare before starting the survey itself:

  • WSU Student ID Number
  • Your First Name
  • Your Last Name
  • WSU Email Address
  • Campus
  • What is your Fall 2026 class standing? (Junior, Senior, etc.)
  • What is your hometown and state? (City, ST)
  • Program or Major in the CESHS
  • Scholarship Name (they will be listed, you choose one or more depending on how many you were awarded)
  • Why did you choose WSU and the College of Education, Sport, and Human Sciences for your education?
  • Please tell the donors/stewards of your scholarships a little bit about yourself.
  • What is the greatest learning experience you’ve had so far in the College of Education, Sport, and Human Sciences?
  • Looking ahead to your time at WSU, what experiences are you looking forward to or are excited about?
  • Are you involved in any campus organizations, sports, activities? If so, what are they?
  • What will receiving this scholarship help you achieve?
  • How do you plan to give back to your community now and in the future?
  • What are your plans, post-graduation?
  • What would you like to say to the donors/steward(s) who provided your scholarship support?
  • Please submit at least 1 photo you feel represents you. Only two images will be allowed, and they must be image format, no PDFs.
  • Would you be open to meeting a donor if interested?
  • Would you be interested in being contacted to share your story?
